Inpatient Hospital See details

Individual and group sessions for outpatient substance abuse are covered with a copay of $10 and $5, respectively, while outpatient blood services are not covered.

Partial Hospitalization See details

Hearing Services includes routine hearing exams with a $20 copay, but fitting/evaluation for hearing aids and prescription hearing aids are not covered. Over-the-counter hearing aids are also not covered.

Vision Services See details

Vision services include coverage for eye exams with a copay of $0-$25, and eyewear with a combined maximum benefit of $100 per year. Routine eye exams have a $20 copay for one visit per year, and contact lenses, eyeglasses (lenses and frames), eyeglass lenses, and eyeglass frames are also covered. Upgrades are not covered.

Dental Services See details

The Geisinger Gold Classic 360 Rx (HMO) plan covers Medicare Dental Services with a $25 copay and other dental services up to a $1500 annual maximum. This plan covers oral exams (2 per year), dental x-rays, and prophylaxis (cleaning) (2 per year). However, fluoride treatment, maxillofacial prosthetics, implant services, prosthodontics fixed, and orthodontics are not covered.
