Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The HealthSpring Achieve (HMO C-SNP) prescription drug plan features an annual drug deductible of $275. For Tier 1 preferred generic drugs, there is no copay when using a preferred pharmacy or preferred mail order service, while standard options require a $9 copay for a one-month supply. Tier 2 generic medications cost as low as a $4 copay for a one-month supply at preferred pharmacies, or no copay for a three-month supply via preferred mail order. Tier 3 preferred brand drugs carry a $47 copay for a one-month supply across all pharmacy and mail order channels. Higher-tier medications require coinsurance, with Tier 4 non-preferred drugs at 50% coinsurance and Tier 5 specialty drugs at 29% coinsurance for a one-month supply.
