After the deductible, you will pay a copay or coinsurance depending on the drug tier and pharmacy. For example, in the initial coverage phase, you'll pay $8 for preferred generic drugs at a standard pharmacy, $47 for standard generic drugs, or $100 for preferred brand drugs. After your total drug costs reach $2000, you enter the catastrophic coverage phase and pay nothing for covered drugs.

Outpatient Services include coverage for all outpatient hospital services, observation services, ambulatory surgical center services, outpatient substance abuse services, and outpatient blood services. Outpatient hospital services have a copay between $0 and $275, observation services have a $275 copay, ambulatory surgical center services have no copay, individual outpatient substance abuse sessions have a copay between $0 and $25, group outpatient substance abuse sessions have a $15 copay, and outpatient blood services have no copay.
Partial Hospitalization is covered by the AARP Medicare Advantage from UHC KC-0003 (HMO-POS) plan. You will pay a $55 copay for this benefit.
Ambulance and Transportation Services are covered by AARP Medicare Advantage from UHC KC-0003 (HMO-POS). Ground and Air Ambulance services have a $275 copay, with no coinsurance. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services has a $140 copay, while Urgently Needed Services has a copay between $0 and $65. Physician specialist services, mental health specialty services, podiatry services, other health care professional, psychiatric services, physical therapy, speech-language pathology services, additional telehealth benefits, and opioid treatment program services are also covered with varying copays.
Preventive services include Medicare-covered services with no copay, an annual physical exam with no copay, and additional preventive services. Additional preventive services include Fitness Benefit and Home and Bathroom Safety Devices and Modifications, both with no copay. Some services are not covered, including Health Education, In-Home Safety Assessment, and others.
Hearing exams are covered with no copay, and routine hearing exams are covered once per year with no copay. Prescription hearing aids are partially covered, with a copay between $199 and $1249 for all types, but not for inner, outer, or over the ear hearing aids. OTC hearing aids are covered with a copay between $99 and $829.
The AARP Medicare Advantage from UHC KC-0003 (HMO-POS) plan covers routine eye exams and eyewear, including contact lenses, eyeglass lenses, and eyeglass frames. Routine eye exams, contact lenses, and eyeglass frames have no copay, while eyeglass lenses have a copay between $0 and $153; this plan does not cover eyeglasses (lenses and frames) or upgrades.
Dental Services are covered, including Medicare Dental Services with 20% coinsurance. Oral Exams, Dental X-Rays, Other Diagnostic Dental Services, Prophylaxis (Cleaning), Fluoride Treatment, and Other Preventive Dental Services have no copay. Restorative Services, Adjunctive General Services, Endodontics, Periodontics, Maxillofacial Prosthetics, and Oral and Maxillofacial Surgery have no copay. Prosthodontics, removable and Prosthodontics, fixed have a coinsurance between 0% and 50%. Implant Services and Orthodontics are not covered.
Home Infusion bundled Services are covered, but require prior authorization. For Medicare Part B Insulin Drugs, there is a $35 copay and a coinsurance between 0% and 20%. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have a coinsurance between 0% and 20%.
Dialysis Services are covered under the AARP Medicare Advantage from UHC KC-0003 (HMO-POS) plan, but prior authorization is required. You will pay 20% coinsurance for these services.
Medical Equipment, including Durable Medical Equipment and Prosthetics/Medical Supplies, is covered by this plan. Durable Medical Equipment has a 20% coinsurance, and Prosthetic Devices and Medical Supplies have a 20% coinsurance with no copay. Diabetic Supplies have no copay, and Diabetic Therapeutic Shoes/Inserts have a 20% coinsurance.
Diagnostic and Radiological Services includes coverage for all diagnostic services with a copay for Medicare-covered diagnostic procedures and tests, as well as a $0 copay for lab services. The plan also covers all radiological services with a copay for Medicare-covered diagnostic and therapeutic radiological services, and a 20% coinsurance for therapeutic radiological services.
Home Health Services are covered by AARP Medicare Advantage from UHC KC-0003 (HMO-POS) with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered, but the plan does not cover Medicare-covered Intensive Cardiac Rehabilitation Services, Medicare-covered Pulmonary Rehabilitation Services, Medicare-covered Supervised Exercise Therapy (SET) for Symptomatic Peripheral Artery Disease (PAD) Services, or Additional Cardiac Rehabilitation Services. Prior authorization is required, and there is a copay.
Skilled Nursing Facility (SNF) services are covered under the AARP Medicare Advantage from UHC KC-0003 (HMO-POS) plan, with a $0 copay for days 1-20 and a $203 copay per day for days 21-100; additional days beyond Medicare-covered and non-Medicare-covered stays are not covered. Prior authorization is required.
The AARP Medicare Advantage from UHC KC-0003 (HMO-POS) plan covers over-the-counter (OTC) items and meal benefits, with no copay for either.
