Monthly Premium

This plan has a $420.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $10100.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $10100.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The AARP Medicare Advantage from UHC NC-0019 (PPO) plan has a $420 deductible for prescription drugs. After you meet your deductible, you will pay a copay or coinsurance for your prescriptions depending on the drug tier and pharmacy. For example, in the initial coverage phase, you will pay no copay for preferred generic drugs at a standard pharmacy. You will pay a $47 copay for standard generic drugs at a standard pharmacy, and a $100 copay for preferred brand drugs at a standard pharmacy.
The AARP Medicare Advantage from UHC NC-0019 (PPO) plan offers a range of benefits, including inpatient hospital stays with a copay, outpatient services with varying copays, and no copay for primary care visits. This plan also covers preventive services, hearing exams, vision services, dental services, and medical equipment, with some services having copays, coinsurance, or prior authorization requirements. Additionally, the plan provides coverage for ambulance, emergency, and home health services, along with skilled nursing facility stays and dialysis services, each with specific cost-sharing arrangements.
Inpatient Hospital benefits are covered, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric services. Additional days for Inpatient Hospital-Acute are covered with no copay, while Non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute, and additional days and Non-Medicare-covered stays for Inpatient Hospital Psychiatric are not covered.
Outpatient Services are covered, including all outpatient hospital services, with a copay ranging from $0 to $385. Ambulatory Surgical Center (ASC) Services and Outpatient Blood Services have no copay, while Outpatient Substance Abuse Services have a copay between $0 and $25 for individual sessions, and a $15 copay for group sessions.
Partial Hospitalization is covered, but requires prior authorization. You will pay a $55 copay for this benefit.
Ambulance and Transportation Services are covered, with prior authorization required for all ambulance services. Ground and air ambulance services have a copay of $265, and there is no coinsurance. Transportation services to health-related locations are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the AARP Medicare Advantage from UHC NC-0019 (PPO) plan. Emergency Services have a $125 copay and no coinsurance, while Urgently Needed Services have a copay between $0 and $55, and no coinsurance. Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation have no copay and no coinsurance.
The AARP Medicare Advantage from UHC NC-0019 (PPO) plan covers primary care physician services with no copay. Chiropractic services have a $20 copay, while occupational therapy services have a copay between $0 and $30. Physician specialist services, mental health specialty services, podiatry services, other health care professional, psychiatric services, physical therapy and speech-language pathology services, and opioid treatment program services have varying copays. Additional telehealth benefits have no copay.
Preventive services include coverage for Medicare-covered services and an annual physical exam with no copay. Additional preventive services, Kidney Disease Education Services, and other preventive services are covered, with some services having a copay, including Fitness Benefit, Medicare-covered Glaucoma Screening, Medicare-covered Diabetes Self-Management Training, Medicare-covered Barium Enemas, Medicare-covered Digital Rectal Exams, and Medicare-covered EKG following Welcome Visit. Other services like health education, in-home safety assessments, and several more are not covered.
Hearing exams are covered with no copay, and routine hearing exams are covered once per year. Prescription hearing aids are covered, with a copay between $199 and $1249, and OTC hearing aids are covered with a copay between $99 and $829. Fitting/evaluation for hearing aids, prescription hearing aids - inner ear, prescription hearing aids - outer ear, and prescription hearing aids - over the ear are not covered.
Vision services include routine eye exams and eyewear. Routine eye exams and contact lenses have no copay, and eyeglass lenses have a copay between $0 and $153, while eyeglass frames have no copay; however, eyeglass lenses and frames are limited to one per two years, and this plan offers a combined maximum of $300 for all eyewear every two years. Eyeglasses (lenses and frames) and upgrades are not covered.
Dental Services are covered, including Medicare Dental Services with 20% coinsurance. Other services like oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, maxillofacial prosthetics, oral and maxillofacial surgery are covered with no copay. Prosthodontics, removable and fixed, have a coinsurance between 0% and 50%. Implants and orthodontics are not covered.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs with a $35 copay and 0-20% coinsurance, and Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs with 0-20% coinsurance. Prior authorization is required for these services.
Dialysis Services are covered by the AARP Medicare Advantage from UHC NC-0019 (PPO) plan. You will pay 20% coinsurance for these services.
Medical equipment, including durable medical equipment, prosthetic devices, and medical supplies, is covered under this plan. Durable medical equipment has a 20% coinsurance, and medical supplies have a 20% coinsurance. Diabetic supplies have no copay, and diabetic therapeutic shoes/inserts have a 20% coinsurance.
Diagnostic and Radiological Services include coverage for all diagnostic services and radiological services, with prior authorization required. Diagnostic Procedures/Tests have a $25 copay, while Lab Services have no copay. Diagnostic Radiological Services have a copay of up to $215, Therapeutic Radiological Services have a $60 copay, and Outpatient X-Ray Services have a $5 copay.
Home Health Services are covered by the AARP Medicare Advantage from UHC NC-0019 (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are not covered by the AARP Medicare Advantage from UHC NC-0019 (PPO) plan. Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, and SET for PAD Services are also not covered.
Skilled Nursing Facility (SNF) services are covered with prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $203 per day. Additional days beyond Medicare-covered and non-Medicare-covered stays for SNF are not covered.
Other Services include a meal benefit with no copay, but acupuncture, over-the-counter items, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
