Vision services include eye exams and eyewear. Eye exams have no copay, and routine eye exams are covered once per year. Eyewear includes contact lenses, eyeglass lenses, and eyeglass frames, all with no copay, but eyeglass lenses are limited to one pair every two years. Eyeglass frames are limited to one pair every two years, and the combined maximum plan benefit coverage amount for all eyewear is $200 every two years.
