This plan has a $420.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $6200.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $6200.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The AARP Medicare Advantage from UHC ST-2 (PPO) plan has an "Enhanced Alternative" drug benefit type. The plan has a deductible of $420. After the deductible, you will pay a copay or coinsurance for your prescriptions, depending on the drug tier and pharmacy. For preferred generic drugs at a standard pharmacy, the copay is $5. Standard generic drugs have a $47 copay at a standard pharmacy. Preferred and standard brand drugs have a $100 copay. Non-preferred drugs have 28% coinsurance.
The AARP Medicare Advantage from UHC ST-2 (PPO) plan offers comprehensive coverage with a variety of benefits. This plan includes no copay for primary care, preventive, and home health services. It also covers inpatient hospital stays, outpatient services, emergency services, hearing, vision, and dental services, with varying copays and coinsurance amounts depending on the service. Additional benefits include coverage for ambulance, partial hospitalization, and home infusion services, as well as diagnostic, radiological, and skilled nursing facility services. There is also coverage for OTC items and meals with no copay. However, some services like transportation, orthodontic, restorative, and private duty nursing are not covered.
Inpatient Hospital benefits include coverage for Inpatient Hospital-Acute and Inpatient Hospital Psychiatric services, with a $330 copay for days 1-5 and no copay for days 6-90. Additional days for Inpatient Hospital-Acute have no copay, while Non-Medicare-covered Stay and Upgrades for Inpatient Hospital-Acute, and Additional Days and Non-Medicare-covered Stay for Inpatient Hospital Psychiatric are not covered.
Outpatient Services include coverage for all outpatient hospital services with a copay between $0 and $330, observation services with a $330 copay, and ambulatory surgical center services with no copay. Outpatient substance abuse services are covered with a copay between $0 and $25 for individual sessions and a $15 copay for group sessions, and outpatient blood services have no copay.
Partial Hospitalization is covered by the AARP Medicare Advantage from UHC ST-2 (PPO) plan, with a $55 copay. Prior authorization is required for this benefit.
Ambulance and Transportation Services are covered, with a $290 copay for both ground and air ambulance services and no coinsurance. Transportation Services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by AARP Medicare Advantage from UHC ST-2 (PPO). Emergency Services have a $140 copay, while Urgently Needed Services have a copay between $0 and $65; there is no coinsurance for either. Worldwide Emergency Services, including Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation, have no copay.
The AARP Medicare Advantage from UHC ST-2 (PPO) plan covers primary care physician services with no copay, chiropractic services with a $20 copay, and occupational therapy services with a copay between $0 and $25. The plan also covers physician specialist services with a copay between $0 and $35, and mental health specialty services with a copay between $0 and $25 for individual sessions and $15 for group sessions. Podiatry services have a $30 copay, other health care professional services have a copay between $0 and $35, and psychiatric services have a copay between $0 and $25 for individual sessions and $15 for group sessions. Additionally, the plan covers physical therapy and speech-language pathology services with a copay between $0 and $25, additional telehealth benefits with no copay, and opioid treatment program services with no copay.
Preventive services are covered under the AARP Medicare Advantage from UHC ST-2 (PPO) plan. The plan covers an annual physical exam with no copay, and other preventive services may have a copay.
Hearing exams are covered with no copay, with routine hearing exams covered once per year. Prescription hearing aids are partially covered, with some types not covered. OTC hearing aids are covered with a copay between $99 and $829.
Vision services include routine eye exams with no copay, and eyewear benefits with a combined maximum of $300 every two years. Eyewear benefits include contact lenses, eyeglass lenses with a copay between $0 and $153, and eyeglass frames with no copay. Eyeglass frames and lenses are limited to one pair every two years. Eyeglasses (lenses and frames) and upgrades are not covered.
Dental Services are covered, including oral exams, dental x-rays, prophylaxis (cleaning), fluoride treatments, and other preventive dental services with no copay. Medicare Dental Services are covered with 20% coinsurance and require prior authorization, while orthodontic services, restorative services, and other dental services are not covered.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs, Medicare Part B Chemotherapy/Radiation Drugs, and Other Medicare Part B Drugs. For Medicare Part B Insulin Drugs, there is a $35 copay and a coinsurance between 0% and 20%. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, there is a coinsurance between 0% and 20%.
Dialysis Services are covered under the AARP Medicare Advantage from UHC ST-2 (PPO) plan, but require prior authorization. The coinsurance for dialysis services is between 20% and 20%.
Medical Equipment benefits include Durable Medical Equipment (DME), Prosthetics/Medical Supplies, and Diabetic Equipment. DME has a 20% coinsurance, and authorization is required. Prosthetic devices and medical supplies have a 20% coinsurance, and diabetic therapeutic shoes and inserts have a 20% coinsurance, while diabetic supplies have no copay.
Diagnostic and Radiological Services include coverage for all diagnostic services and all radiological services, with prior authorization required. Diagnostic Procedures/Tests have a $45 copay, while Lab Services have no copay. Diagnostic Radiological Services have a copay of up to $200, and Therapeutic Radiological Services have a coinsurance of at least 20%. Outpatient X-Ray Services have a $25 copay.
Home Health Services are covered under the AARP Medicare Advantage from UHC ST-2 (PPO) plan with no copay and no coinsurance. However, Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered by the AARP Medicare Advantage from UHC ST-2 (PPO) plan, but specific services including Cardiac Rehabilitation Services, Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, and SET for PAD Services are not covered. Prior authorization is required for this benefit.
Skilled Nursing Facility (SNF) services are covered by the AARP Medicare Advantage from UHC ST-2 (PPO) plan. There is no copay for days 1-20, and a $203 copay per day for days 21-100, but additional days beyond Medicare-covered for SNF and non-Medicare-covered SNF stays are not covered.
Other Services includes coverage for Over-the-Counter (OTC) items and meal benefits with no copay, while acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
