The AARP Medicare Advantage from UHC ID-0003 (PPO) plan has a $420 deductible for prescription drugs. After the deductible is met, you will pay a copay or coinsurance depending on the drug tier and pharmacy. For example, you will pay a $5 copay for preferred generic drugs at a standard pharmacy, and $100 for preferred brand drugs. Non-preferred drugs have a 28% coinsurance. Once your total drug costs reach $2000, you enter the catastrophic coverage phase, where you pay nothing for covered drugs.
The AARP Medicare Advantage from UHC ID-0003 (PPO) plan offers comprehensive coverage, including no copay for primary care physician services, hearing exams, eye exams, and many outpatient services. Inpatient hospital stays have a copay of $350 for the first few days, and emergency services have a $125 copay. This plan also includes coverage for dental, vision, and hearing services. Outpatient Services include coverage for all outpatient hospital services, with a copay between $0 and $350, and observation services with a $350 copay. Additionally, this plan covers Ambulatory Surgical Center (ASC) Services and Outpatient Blood Services with no copay, and outpatient substance abuse services with a copay between $0 and $25 for individual sessions, and a $15 copay for group sessions.
Partial Hospitalization is covered by this plan with a $55 copay, and prior authorization is required.
Ambulance and Transportation Services are covered under the AARP Medicare Advantage from UHC ID-0003 (PPO) plan. Ground and air ambulance services have a copay of $275, with no coinsurance, while transportation services to any health-related location are not covered.

Preventive services include no copay for an annual physical exam, and no copay for additional preventive services like glaucoma screenings, diabetes self-management training, barium enemas, digital rectal exams, and EKG following a welcome visit. Health education, in-home safety assessments, personal emergency response systems, medical nutrition therapy, post-discharge in-home medication reconciliation, re-admission prevention, wigs for hair loss related to chemotherapy, weight management programs, alternative therapies, therapeutic massage, adult day health services, nutritional/dietary benefits, home-based palliative care, in-home support services, support for caregivers of enrollees, additional sessions of smoking and tobacco cessation counseling, enhanced disease management, telemonitoring services, and counseling services are not covered.
Hearing exams are covered with no copay, and routine hearing exams are covered once per year with no copay. Prescription hearing aids are covered with a copay between $199 and $1249, and OTC hearing aids are covered with a copay between $99 and $829. Fitting/evaluation for hearing aids, and prescription hearing aids for inner ear, outer ear, and over the ear are not covered.
Vision services include eye exams and eyewear. Eye exams have no copay, and routine eye exams are covered once per year. Eyewear includes contact lenses, eyeglass lenses, and eyeglass frames, all with no copay; however, eyeglass lenses are covered up to $153.00 and frames are covered once every two years.
Dental services include coverage for Medicare dental services with 20% coinsurance, other dental services, and other preventive services with a $1,500 annual maximum. Oral exams, dental X-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, and other preventive dental services have no copay. Restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), maxillofacial prosthetics, and oral and maxillofacial surgery have no copay, but may have a coinsurance of up to 50%. Orthodontic and implant services are not covered.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs, Medicare Part B Chemotherapy/Radiation Drugs, and Other Medicare Part B Drugs, with a $35 copay for Medicare Part B Insulin Drugs. Coinsurance for the listed services ranges from 0% to 20%.
Dialysis Services are covered, but require prior authorization. You will pay 20% coinsurance for these services.
Medical Equipment is covered, including Durable Medical Equipment (DME) with 20% coinsurance and Prosthetics/Medical Supplies with 20% coinsurance. Diabetic Supplies have no copay, and Diabetic Therapeutic Shoes/Inserts have 20% coinsurance.
Diagnostic and Radiological Services include coverage for diagnostic procedures and tests with a $50 copay, lab services with no copay, diagnostic radiological services with a copay of at most $225, therapeutic radiological services with at most 20% coinsurance, and outpatient X-ray services with a $25 copay. All services require prior authorization.
Home Health Services are covered with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered with prior authorization, but the plan does not cover Cardiac Rehabilitation Services, Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, or SET for PAD Services. There is a copay for these services, but the exact amount is not specified.
Skilled Nursing Facility (SNF) services are covered, but require prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $203.00. Additional days beyond Medicare-covered and non-Medicare-covered stays for SNF are not covered.
The AARP Medicare Advantage from UHC ID-0003 (PPO) plan covers Over-the-Counter (OTC) Items and Meal Benefits. Over-the-Counter (OTC) Items have no copay, and Meal Benefits also have no copay.
