Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $10100.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $10100.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The AARP Medicare Advantage from UHC AZ-0006 (PPO) plan has a $420 deductible for prescription drugs. After the deductible is met, you will pay a copay or coinsurance for your prescriptions depending on the drug tier and pharmacy. For example, you will pay a $10 copay for preferred generic drugs at a standard pharmacy, and a $100 copay for preferred brand drugs. Once your total drug costs reach $2,000, you enter the catastrophic coverage phase where you pay nothing for your Part D covered drugs.
The AARP Medicare Advantage from UHC AZ-0006 (PPO) plan offers a range of benefits with varying costs. It covers inpatient hospital stays with a copay for the initial days, and outpatient services with copays depending on the service. There are also no copays for primary care, preventive services, vision exams, hearing exams, and many dental services. This plan includes coverage for emergency services, ambulance services, and home health services. Additionally, the plan covers hearing aids, eyewear, and dental services, as well as medical equipment like durable medical equipment. The plan also covers home infusion, dialysis, and skilled nursing facility services.
Inpatient Hospital benefits are covered, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric. For Inpatient Hospital-Acute, you pay a $365 copay for days 1-6, and no copay for days 7-90, and no copay for days 91-999. For Inpatient Hospital Psychiatric, you pay a $365 copay for days 1-4, and no copay for days 5-90. Non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute are not covered, and Additional Days for Inpatient Hospital Psychiatric, and Non-Medicare-covered stays for Inpatient Hospital Psychiatric are not covered.
Outpatient Services, including all outpatient hospital services, observation services, ambulatory surgical center services, outpatient substance abuse services, and outpatient blood services, are covered. Outpatient Hospital Services have a copay between $0 and $365, Observation Services have a copay of $365, and Ambulatory Surgical Center Services and Outpatient Blood Services have no copay, while Individual Sessions for Outpatient Substance Abuse have a copay between $0 and $25, and Group Sessions for Outpatient Substance Abuse have a copay of $15.
Partial Hospitalization is covered by the AARP Medicare Advantage from UHC AZ-0006 (PPO) plan. You will have a $55 copay for this benefit.
Ambulance and Transportation Services are covered, with prior authorization required for all ambulance services. Ground and Air Ambulance Services each have a $275 copay, and there is no coinsurance. Transportation Services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services have a $125 copay, and no coinsurance, while Urgently Needed Services have a copay between $0 and $55, and no coinsurance. Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation all have no copay and no coinsurance.
Primary Care Physician Services are covered with no copay, Chiropractic Services are covered with a $20 copay, and Occupational Therapy Services have a copay between $0 and $30. Physician Specialist Services, Mental Health Specialty Services, Podiatry Services, Other Health Care Professional, Psychiatric Services, Physical Therapy and Speech-Language Pathology Services, Additional Telehealth Benefits, and Opioid Treatment Program Services are covered with varying copays. Routine Chiropractic Care is not covered.
Preventive services include Medicare-covered preventive services with no copay, and an annual physical exam with no copay. Additional preventive services, like health education, are not covered. Other preventive services, like Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit, are covered with no copay.
Hearing exams are covered with no copay, and routine hearing exams are covered once per year with no copay. Prescription hearing aids are covered with a copay between $199 and $1249, and OTC hearing aids are covered with a copay between $99 and $829. Fitting/evaluation for hearing aids, prescription hearing aids - inner ear, prescription hearing aids - outer ear, and prescription hearing aids - over the ear are not covered.
Vision Services include eye exams and eyewear. Eye exams have no copay, and include routine eye exams. Eyewear includes contact lenses, eyeglass lenses, and eyeglass frames, but does not cover eyeglasses (lenses and frames) or upgrades. Contact lenses, eyeglass lenses, and eyeglass frames all have a $0 copay. There is a combined maximum plan benefit of $300 for eyewear, and the plan covers one pair of eyeglass lenses and one pair of eyeglass frames every two years.
Dental services from AARP Medicare Advantage from UHC AZ-0006 (PPO) include coverage for Medicare Dental Services with a 20% coinsurance and other dental services. Oral exams, dental x-rays, prophylaxis (cleaning), fluoride treatments, and other preventive dental services have no copay. However, orthodontic services, restorative services, and other services are not covered.
Home Infusion bundled Services, including Medicare Part B Insulin Drugs, are covered with a $35 copay and a coinsurance between 0% and 20%, depending on the specific drug. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs are also covered, with a coinsurance between 0% and 20%.
Dialysis Services are covered by the AARP Medicare Advantage from UHC AZ-0006 (PPO) plan, but prior authorization is required. You will pay 20% coinsurance.
Medical Equipment benefits include Durable Medical Equipment (DME) with 20% coinsurance and Prosthetic Devices with 20% coinsurance. Diabetic Supplies have no copay, and Diabetic Therapeutic Shoes/Inserts have 20% coinsurance.
Diagnostic and Radiological Services include coverage for all diagnostic services with a $50 copay, lab services with no copay, and all radiological services. Diagnostic Radiological Services have a copay of up to $250, while Therapeutic Radiological Services have a coinsurance of up to 20%, and Outpatient X-Ray Services have a $25 copay.
Home Health Services are covered by the AARP Medicare Advantage from UHC AZ-0006 (PPO) plan with no copay and no coinsurance. However, additional hours of care and personal care services are not covered.
Cardiac Rehabilitation Services are covered, but the specific services including Cardiac Rehabilitation, Intensive Cardiac Rehabilitation, Pulmonary Rehabilitation, and SET for PAD Services, are not covered. Prior authorization is required, and there is a copay.
Skilled Nursing Facility (SNF) services are covered, but require prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $203. Additional days beyond Medicare-covered for SNF and non-Medicare-covered stays for SNF are not covered.
Other Services includes Over-the-Counter (OTC) Items with no copay. Acupuncture, Meal Benefit, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
