Monthly Premium
The Monthly Premium for this plan is $38.40. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $1.30. You must continue to pay paying your reduced Part B Premium.
Deductibles

Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $13900.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $13900.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The UHC PathWays Dual Care IN-S1 (PPO D-SNP) prescription drug plan features an annual drug deductible of $615. For Tier 1 preferred generic drugs, members pay no copay for a 1-month or 3-month supply at standard pharmacies, as well as no copay for a 3-month supply through standard mail order. Tier 2 generic drugs require a 25% coinsurance for both 1-month and 3-month fills at standard pharmacies and standard mail order. Tier 3 preferred brand drugs are also subject to a 25% coinsurance for 1-month and 3-month supplies through standard pharmacies and mail order. Additionally, Tier 4 non-preferred drugs and Tier 5 specialty drugs require a 25% coinsurance for a 1-month supply from standard pharmacies or standard mail order. This plan provides clear, tier-based pricing to help you manage your healthcare budget.

Prescription hearing aids (excluding inner ear, outer ear, and over the ear types) up to a $2,200 limit and up to two OTC hearing aids are covered every two years with no copay or coinsurance.
UHC PathWays Dual Care IN-S1 (PPO D-SNP) partially covers vision services with no copay and no coinsurance, which includes one routine eye exam per year and a $250 annual limit for contacts, eyeglass lenses, and frames. These services require prior authorization and carry a 20% coinsurance with no copay.
Skilled Nursing Facility (SNF) services are covered by UHC PathWays Dual Care IN-S1 (PPO D-SNP) with no copay or coinsurance, and prior authorization is required. The plan does not require a 3-day prior hospital stay for admission, but additional days beyond the Medicare-covered limit are not covered.
Other Services are partially covered by UHC PathWays Dual Care IN-S1 (PPO D-SNP), featuring Over-the-Counter (OTC) items and meal benefits for chronic illnesses with no copay and no coinsurance. Acupuncture is not covered under this benefit, and prior authorization is required for the meal benefit.
