Monthly Premium
The Monthly Premium for this plan is $47.40. This is the amount you must pay every month.
This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has a $520.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $8950.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $8950.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Wellcare Dual Reserve Open (PPO D-SNP) plan has a $520 deductible for prescription drugs. In the initial coverage phase, after you meet your deductible, you will pay a copay or coinsurance depending on the drug tier and the pharmacy you use. For example, preferred generic drugs have a $19 copay at preferred pharmacies, while specialty tier drugs have no copay. Once your total drug costs reach $2000, you enter the catastrophic coverage phase, where you will pay nothing for covered drugs.
The Wellcare Dual Reserve Open (PPO D-SNP) plan offers a range of benefits with varying costs. Inpatient hospital stays have a copay, but some days are covered with no copay. Outpatient, emergency, and primary care services have copays, while preventive services, routine hearing and vision exams, and dental services have no copay. This plan also covers ambulance and transportation services, with copays for ground and air ambulance. Other benefits include coverage for home health services, medical equipment, and diagnostic services with varying copays and coinsurance. The plan covers some services with no copay, such as over-the-counter items, meal benefits, and some vision and dental services.
Inpatient Hospital benefits are covered under the Wellcare Dual Reserve Open (PPO D-SNP) plan. For Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, you will pay a $250 copay for days 1-5, and no copay for days 6-90, with no coinsurance. Additional days and non-Medicare covered stays for both Inpatient Hospital-Acute and Inpatient Hospital Psychiatric are not covered.
Outpatient Services include coverage for all outpatient hospital services with a copay between $0 and $200, observation services with a copay between $125 and $200, ambulatory surgical center services with a $150 copay, and outpatient substance abuse services with a $25 copay for both individual and group sessions. Outpatient blood services are covered with no copay.
Partial Hospitalization is covered by the Wellcare Dual Reserve Open (PPO D-SNP) plan, but requires prior authorization. The copay for this benefit is $105.
Ambulance and Transportation Services are covered under the Wellcare Dual Reserve Open (PPO D-SNP) plan. Ground and air ambulance services have a $280 copay, while transportation services to a plan-approved health-related location have no copay and are limited to 36 one-way trips per year.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Coverage are covered by the Wellcare Dual Reserve Open (PPO D-SNP) plan. Emergency Services and Worldwide Emergency Coverage have a $125 copay, and Urgently Needed Services have a $40 copay. Worldwide Emergency Transportation is not covered.
The Wellcare Dual Reserve Open (PPO D-SNP) plan covers primary care physician services with no copay, chiropractic services with a $20 copay, occupational therapy services with a $25 copay, and specialist services with a $25 copay. Mental health specialty services, psychiatric services, and opioid treatment program services have a $25 copay for individual and group sessions, and physical therapy and speech-language pathology services have a $25 copay. Additional telehealth benefits have a copay between $0 and $40. Podiatry services are not covered.
Preventive services, including annual physical exams, are covered with no copay. Additional preventive services, including Personal Emergency Response System (PERS), Alternative Therapies, Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, EKG following Welcome Visit, and Fitness Benefit, are covered with no copay. Kidney Disease Education Services are covered with 20% coinsurance. Health Education, In-Home Safety Assessment, Medical Nutrition Therapy (MNT), Post discharge In-Home Medication Reconciliation, Re-admission Prevention, Wigs for Hair Loss Related to Chemotherapy, Weight Management Programs, Therapeutic Massage, Adult Day Health Services, Nutritional/Dietary Benefit, Home-Based Palliative Care, In-Home Support Services, Support for Caregivers of Enrollees, Additional Sessions of Smoking and Tobacco Cessation Counseling, Enhanced Disease Management, Telemonitoring Services, Home and Bathroom Safety Devices and Modifications, and Counseling Services are not covered.
Hearing services include coverage for hearing exams with a $25 copay, routine hearing exams with no copay, and fitting/evaluation for hearing aids with no copay. Prescription hearing aids are covered up to $1000 per year, but prescription hearing aids for the inner ear, outer ear, and over the ear are not covered, and OTC hearing aids are not covered.
The Wellcare Dual Reserve Open (PPO D-SNP) plan covers vision services, including eye exams with a copay of $0-$25, and eyewear with a combined maximum of $300 per year with no copay. Routine eye exams are covered with no copay for one visit every year, and contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades are all covered with no copay.
The Wellcare Dual Reserve Open (PPO D-SNP) plan covers Medicare Dental Services with a $25 copay. Other dental services include oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), and oral and maxillofacial surgery, all with no copay, but not maxillofacial prosthetics, implant services, or orthodontics. Orthodontic services are covered up to a maximum of $4,000 per year.
Home Infusion bundled Services are covered, with prior authorization required. For Medicare Part B insulin drugs, there is a $35 copay. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have a coinsurance between 0% and 20%.
Dialysis Services are covered under the Wellcare Dual Reserve Open (PPO D-SNP) plan. You will pay 20% coinsurance for these services.
Medical equipment, including durable medical equipment, prosthetics, medical supplies, and diabetic equipment, is covered by the Wellcare Dual Reserve Open (PPO D-SNP) plan. Durable Medical Equipment (DME) has a 20% coinsurance with no copay, and requires authorization. Prosthetic devices have a 20% coinsurance with no copay, while medical supplies have a 20% coinsurance with no copay. Diabetic supplies have no copay, and the plan covers diabetic therapeutic shoes/inserts with a 20% coinsurance.
Diagnostic and Radiological Services are covered, including all diagnostic services and radiological services. Diagnostic Procedures/Tests and Lab Services have no copay, while Diagnostic Radiological Services have a copay of at most $200.00 and Outpatient X-Ray Services have a copay of $45.00. Therapeutic Radiological Services have a coinsurance of at least 20%.
Home Health Services are covered by the Wellcare Dual Reserve Open (PPO D-SNP) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered, but none of the sub-services are covered. The copay for Cardiac Rehabilitation Services is listed in the plan details.
Skilled Nursing Facility (SNF) benefits are covered by the Wellcare Dual Reserve Open (PPO D-SNP) plan, but require prior authorization. For days 1-20 and 51-100, there is no copay, but days 21-50 have a $214 copay. Additional days beyond Medicare and non-Medicare-covered stays are not covered.
The Wellcare Dual Reserve Open (PPO D-SNP) plan covers over-the-counter (OTC) items and meal benefits, with no copay for either. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
