Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $14000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $14000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by AARP Medicare Advantage Patriot No Rx NC-MA01 (PPO).

You'll find no copays for primary care, preventive services like exams and screenings, and many dental services. Prior authorization is required for all services.
Partial Hospitalization is covered by the AARP Medicare Advantage Patriot No Rx NC-MA01 (PPO) plan, with a $55 copay. Prior authorization is required for this benefit.

This plan also covers OTC hearing aids with a copay between $99 and $829, but does not cover fitting/evaluation for hearing aids, or prescription hearing aids - inner ear, outer ear, or over the ear.
Vision services include eye exams and eyewear. Eye exams have no copay, and routine eye exams are covered annually. Eyewear includes contact lenses, eyeglass lenses, and eyeglass frames with a combined maximum of $250 every two years, and contact lenses have no copay. Eyeglasses and upgrades are not covered.
The AARP Medicare Advantage Patriot No Rx NC-MA01 (PPO) plan covers dental services, including oral exams, dental x-rays, other diagnostic services, cleanings, fluoride treatments, other preventive services, restorative services, and adjunctive general services with no copay. The plan has a 20% coinsurance for Medicare dental services and a maximum benefit of $1,500 per year for other dental services. Prosthodontics, removable and fixed, have a coinsurance between 0% and 50%. The plan does not cover implant services or orthodontics.
Home Infusion bundled Services are covered by the AARP Medicare Advantage Patriot No Rx NC-MA01 (PPO) plan, including Medicare Part B Insulin Drugs with a $35 copay and 0-20% coinsurance, and Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs with 0-20% coinsurance. Prior authorization is required for this benefit.
Dialysis Services are covered under the AARP Medicare Advantage Patriot No Rx NC-MA01 (PPO) plan. You will pay 20% coinsurance for these services.
Medical Equipment benefits include Durable Medical Equipment (DME), Prosthetics/Medical Supplies, and Diabetic Equipment. DME has a 20% coinsurance and requires authorization, while DME for use outside the home is not covered. Prosthetic devices have a 20% coinsurance, and medical supplies have a 20% coinsurance. Diabetic supplies have no copay, and diabetic therapeutic shoes/inserts have a 20% coinsurance.
Diagnostic and Radiological Services are covered, including diagnostic procedures and tests with a $15 copay, lab services with no copay, diagnostic radiological services with a copay up to $225, therapeutic radiological services with a copay up to $60, and outpatient X-ray services with a $5 copay.
Home Health Services are covered under the AARP Medicare Advantage Patriot No Rx NC-MA01 (PPO) plan with no copay and no coinsurance. Additional hours of care and personal care services are not covered.
Cardiac Rehabilitation Services are covered, but the plan does not cover Medicare-covered Intensive Cardiac Rehabilitation Services, Medicare-covered Pulmonary Rehabilitation Services, Medicare-covered Supervised Exercise Therapy (SET) for Symptomatic Peripheral Artery Disease (PAD) Services, or Additional Cardiac Rehabilitation Services. A copay applies, but the specific amount is not provided.
Skilled Nursing Facility (SNF) services are covered with prior authorization required. There is no copay for days 1-20, and a $203 copay for days 21-100. Additional days beyond Medicare-covered for SNF and non-Medicare-covered SNF stays are not covered.
Other Services offered by the AARP Medicare Advantage Patriot No Rx NC-MA01 (PPO) plan include a meal benefit with no copay, but acupuncture, over-the-counter items, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
