Once your total out-of-pocket costs reach $10100.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The AARP Medicare Advantage from UHC TN-0001 (PPO) plan has an "Enhanced Alternative" drug benefit. The plan has a $255 deductible for prescription drugs. During the initial coverage phase, you'll pay varying copays or coinsurance depending on the drug tier and pharmacy. For example, you will pay a $0 copay for preferred generic drugs at standard pharmacies. You will pay a $100 copay for preferred brand drugs at preferred mail pharmacies. Once your total drug costs reach $2000, you enter the catastrophic coverage phase where you pay nothing for covered drugs.
The AARP Medicare Advantage from UHC TN-0001 (PPO) plan offers a wide range of benefits, including inpatient and outpatient hospital services, with varying copays. You can expect no copay for primary care, hearing exams, eye exams, dental cleanings, and home health services. The plan also covers emergency services, and offers no copay for Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation. Inpatient Hospital benefits are covered, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, both of which require prior authorization. For Inpatient Hospital-Acute, you pay a $395 copay for days 1-5, and no copay for days 6-90, with no coinsurance; for Additional Days, there is no copay and no coinsurance. For Inpatient Hospital Psychiatric, you pay a $395 copay for days 1-4, and no copay for days 5-90, with no coinsurance. Non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute are not covered, and Additional Days and Non-Medicare-covered stays for Inpatient Hospital Psychiatric are not covered.
Outpatient Services include coverage for all outpatient hospital services, with copays ranging from $0 to $395, and observation services with a $395 copay. Ambulatory Surgical Center (ASC) Services and Outpatient Blood Services have no copay, and Outpatient Substance Abuse Services have a copay of $0-$25 for individual sessions and $15 for group sessions.
Partial Hospitalization is covered by the AARP Medicare Advantage from UHC TN-0001 (PPO) plan, with a $55 copay. Prior authorization is required for this benefit.
Ambulance and Transportation Services are covered under the AARP Medicare Advantage from UHC TN-0001 (PPO) plan. Ground and Air Ambulance Services have a $290 copay, with no coinsurance, while transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services have a $125 copay, while Urgently Needed Services have a copay between $0 and $55. Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation all have no copay. There is no coinsurance for any of these services.
The AARP Medicare Advantage from UHC TN-0001 (PPO) plan covers primary care physician services with no copay, chiropractic services with a $20 copay, occupational therapy services with a copay between $0 and $35, and physician specialist services with a copay between $0 and $35. This plan also covers mental health specialty services with a copay between $0 and $25 for individual sessions and a $15 copay for group sessions, podiatry services with a $25 copay, and other health care professional services with a copay between $0 and $35. Psychiatric services, physical therapy, speech-language pathology services, additional telehealth benefits, and opioid treatment program services are covered with varying copays.
Preventive Services include an annual physical exam with no copay, and additional preventive services, kidney disease education services, and other preventive services with a copay. Health education, in-home safety assessments, personal emergency response systems, medical nutrition therapy, post-discharge in-home medication reconciliation, readmission prevention, wigs for hair loss related to chemotherapy, weight management programs, alternative therapies, therapeutic massage, adult day health services, nutritional/dietary benefits, home-based palliative care, in-home support services, support for caregivers of enrollees, additional sessions of smoking and tobacco cessation counseling, enhanced disease management, telemonitoring services, remote access technologies, home and bathroom safety devices and modifications, and counseling services are not covered. Glaucoma screenings, diabetes self-management training, barium enemas, digital rectal exams, and EKG following a Welcome Visit have no copay.
Hearing services include hearing exams, which have no copay, and prescription hearing aids, which have a copay between $199 and $1249. Fitting/evaluation for hearing aids, prescription hearing aids - inner ear, prescription hearing aids - outer ear, and prescription hearing aids - over the ear are not covered.
The AARP Medicare Advantage from UHC TN-0001 (PPO) plan covers vision services, including eye exams with no copay, and eyewear with no copay for contact lenses and eyeglass frames, but has a $0-$153 copay for eyeglass lenses. Eyeglass frames and lenses are limited to one pair every two years, while the plan offers a combined maximum of $200 for all eyewear every two years.
Dental Services are covered, with a 20% coinsurance for Medicare Dental Services. Oral exams, dental x-rays, prophylaxis (cleaning), fluoride treatments, and other preventive dental services are covered with no copay, but have limits on the number of visits. Orthodontic services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), maxillofacial prosthetics, implant services, oral and maxillofacial surgery, and orthodontics are not covered.
Home Infusion bundled Services are covered, including insulin and other Medicare Part B drugs. For Medicare Part B Insulin Drugs, there is a $35 copay and a coinsurance between 0% and 20%. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, there is a coinsurance between 0% and 20%.
Dialysis Services are covered, but require prior authorization. You will pay 20% coinsurance for these services.
Medical Equipment is covered, including Durable Medical Equipment (DME) with 20% coinsurance and Prosthetics/Medical Supplies - Non-Medicare benefit with coinsurance. Diabetic Equipment is covered, including Diabetic Supplies with no copay and Diabetic Therapeutic Shoes/Inserts with 20% coinsurance.
Diagnostic and Radiological Services include coverage for Diagnostic Procedures/Tests with a $25 copay, Lab Services with no copay, Diagnostic Radiological Services with a copay up to $225, Therapeutic Radiological Services with a minimum $80 copay, and Outpatient X-Ray Services with a $10 copay. All services require prior authorization.
Home Health Services are covered by the AARP Medicare Advantage from UHC TN-0001 (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered, but Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, and SET for PAD Services are not covered. Prior authorization is required for this benefit.
Skilled Nursing Facility (SNF) services are covered by the AARP Medicare Advantage from UHC TN-0001 (PPO) plan, but require prior authorization. You will have no copay for days 1-20, and a $203 copay for days 21-100. Additional days beyond Medicare-covered for SNF and non-Medicare-covered stays for SNF are not covered.
The AARP Medicare Advantage from UHC TN-0001 (PPO) plan covers a meal benefit with no copay, but acupuncture, over-the-counter items, Dual Eligible SNPs with Highly Integrated Services, and other services like Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services are not covered. Other services such as private duty nursing, case management, and home and community based services are not covered.
