This plan has a $255.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $10100.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $10100.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The AARP Medicare Advantage from UHC NC-0001 (PPO) plan has a $255.00 deductible for prescription drugs. After the deductible is met, you will pay the following costs. For standard generic drugs, you will pay a $47.00 copay, while preferred brand drugs have a $100.00 copay. Non-preferred drugs will have a 30% coinsurance. Once your total yearly out-of-pocket drug costs reach $2000.00, you will enter the catastrophic coverage phase and pay nothing for Part D covered drugs.

Non-Medicare-covered stays and upgrades are not covered.
Outpatient Services include coverage for Outpatient Hospital Services with a copay between $0 and $375, Observation Services with a $375 copay, Ambulatory Surgical Center (ASC) Services with no copay, Outpatient Substance Abuse Services with a copay between $0 and $25 for individual sessions, and a $15 copay for group sessions, and Outpatient Blood Services with no copay. Prior authorization is required for all services.
Partial Hospitalization is covered, but requires prior authorization, and has a $55 copay.
Ambulance and Transportation Services are covered, with prior authorization required for all ambulance services. Ground and air ambulance services have a $225 copay, and there is no coinsurance. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services have a $125 copay, and Urgently Needed Services have a copay between $0 and $55. Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation have no copay.
The AARP Medicare Advantage from UHC NC-0001 (PPO) plan covers primary care physician services with no copay, chiropractic services with a $20 copay, and occupational therapy services with a copay between $0 and $35. Physician specialist services, mental health specialty services, podiatry services, other health care professional services, psychiatric services, physical therapy and speech-language pathology services, additional telehealth benefits, and opioid treatment program services are also covered, with varying copays. Routine chiropractic care is not covered.
Preventive services include an annual physical exam with no copay, and additional preventive services that may have a copay, including Fitness Benefit. Kidney Disease Education Services, Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit are covered with no copay. Other services like Health Education, In-Home Safety Assessment, and more are not covered.
Hearing exams are covered with no copay, but fitting/evaluation for hearing aids are not covered. Prescription hearing aids are covered with a copay between $199 and $1249 depending on the type of hearing aid. OTC hearing aids are covered with a copay between $99 and $829.
The AARP Medicare Advantage from UHC NC-0001 (PPO) plan covers vision services, including routine eye exams and eyewear. Eye exams and contact lenses have no copay, while eyeglass lenses have a copay of $0-$153, and eyeglass frames have no copay. Eyeglasses (lenses and frames) and upgrades are not covered.
The AARP Medicare Advantage from UHC NC-0001 (PPO) plan covers dental services, including oral exams, dental x-rays, prophylaxis (cleaning), fluoride treatments, and other preventive services, with no copay for these services. However, orthodontic services, restorative services, and other dental services are not covered.
Home Infusion bundled Services are covered, and require prior authorization. For Medicare Part B Insulin Drugs, there is a $35 copay and a coinsurance between 0% and 20%. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, there is a coinsurance between 0% and 20%.
Dialysis Services are covered, but require prior authorization. The coinsurance for dialysis services is 20%.
Medical Equipment benefits include Durable Medical Equipment (DME), Prosthetics/Medical Supplies, and Diabetic Equipment. DME has a 20% coinsurance, while Durable Medical Equipment for use outside the home is not covered. Prosthetic Devices and Medical Supplies have a 20% coinsurance, while Diabetic Supplies have no copay, and Diabetic Therapeutic Shoes/Inserts have a 20% coinsurance.
Diagnostic and Radiological Services are covered, with prior authorization required. Diagnostic Procedures/Tests have a $25 copay, and Lab Services have no copay. Diagnostic Radiological Services have a copay of at most $85, Therapeutic Radiological Services have a copay of at least $60, and Outpatient X-Ray Services have a $10 copay.
Home Health Services are covered under the AARP Medicare Advantage from UHC NC-0001 (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered with this AARP Medicare Advantage plan, but the specific services are not covered. Prior authorization is required for this benefit.
Skilled Nursing Facility (SNF) services are covered by the AARP Medicare Advantage from UHC NC-0001 (PPO) plan, but require prior authorization. There is no copay for days 1-20, and a $203 copay for days 21-100. Additional days beyond Medicare-covered and non-Medicare-covered stays for SNF are not covered.
Other Services include a meal benefit with no copay, but acupuncture, over-the-counter items, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
