Additional days for Inpatient Hospital-Acute, starting from day 91, have no copay. Non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute, and additional days and non-Medicare-covered stays for Inpatient Hospital Psychiatric are not covered.

Outpatient Services, including all outpatient hospital services and outpatient blood services, are covered. Hearing Services include hearing exams, routine hearing exams, fitting/evaluation for hearing aids, prescription hearing aids, and OTC hearing aids. Hearing exams have a $15 copay, and routine hearing exams and fitting/evaluation for hearing aids have no copay. Prescription hearing aids (all types) have a copay between $199 and $499, but prescription hearing aids - inner ear, outer ear, and over the ear are not covered, and OTC hearing aids are not covered.

Vision Services See details

Vision services are covered, including eye exams and eyewear. Eye exams have a copay of $0-$15, while routine eye exams have no copay. Eyewear is covered with no copay, with a combined maximum of $500 per year for all eyewear, but eyeglass lenses, frames, and upgrades are not covered.

Dental Services See details

Dental Services include Medicare Dental Services with a $15 copay, and other dental services with a $3,000 annual maximum. Oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), and other preventive dental services are covered with no copay, but fluoride treatment, maxillofacial prosthetics, and implant services are not covered. Skilled Nursing Facility (SNF) services are covered, but require prior authorization. For days 1-20, the copay is $20, and for days 21-100, the copay is $214. Additional days beyond Medicare-covered and non-Medicare-covered stays for SNF are not covered.

Other Services See details

The Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan covers acupuncture with a $15 copay, up to 20 treatments per year with prior authorization. Over-the-counter (OTC) items are covered up to $900 per year, and the plan also covers a meal benefit with no copay.
