Monthly Premium

Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $9350.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

After you meet the deductible, your costs will vary depending on the specific drug tier and pharmacy you use, with the details of these costs not provided in this snippet. Once your total drug costs reach $2000, you enter the next coverage phase. If you qualify for the low-income subsidy (LIS), you'll pay $55.60. After your yearly out-of-pocket drug costs reach $2000, you pay nothing for Medicare Part D covered drugs.

Ambulance and Transportation Services are covered, including ground and air ambulance services with a 20% coinsurance. Transportation Services to a plan-approved health-related location are covered with no copay and up to 96 one-way trips per year via rideshare, bus/subway, van, or medical transport.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Healthy Blue Dual Advantage 2 (HMO D-SNP) plan. Emergency Services have a $90 copay, Urgently Needed Services have a $20 copay, and Worldwide Emergency Services have a $0 copay for Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation.
The Healthy Blue Dual Advantage 2 (HMO D-SNP) plan covers Primary Care Physician Services, Chiropractic Services, Occupational Therapy, Physician Specialist Services, Mental Health Specialty Services, Podiatry Services, Other Health Care Professional, Psychiatric Services, Physical Therapy and Speech-Language Pathology Services, Additional Telehealth Benefits, and Opioid Treatment Program Services. Primary Care Physician Services, Physician Specialist Services, and Physical Therapy and Speech-Language Pathology Services have a 20% coinsurance, while Routine Chiropractic Care and Additional Telehealth Benefits have no copay.
Preventive services include coverage for annual physical exams with no copay, additional preventive services with a copay, and other services such as glaucoma screening, diabetes self-management training, barium enemas, digital rectal exams, and EKG following a welcome visit, each with a 20% coinsurance. Health education, in-home safety assessments, and several other services are not covered.
Hearing exams are covered with a coinsurance of at most 20% for routine exams, and no copay for Medicare-covered benefits and fitting/evaluation for hearing aids, while prescription hearing aids are covered with no copay and a yearly maximum benefit of $3,000. OTC hearing aids are covered with no copay and a yearly maximum benefit of $300.
Vision Services include coverage for eye exams and eyewear. Eye exams have a 20% coinsurance, while routine eye exams have no copay. Eyewear has a 20% coinsurance, and contact lenses, eyeglasses (lenses and frames), and eyeglass lenses have no copay. Upgrades are not covered.
The Healthy Blue Dual Advantage 2 (HMO D-SNP) plan covers Medicare Dental Services with 20% coinsurance, and other dental services with a $4,000 annual maximum. Oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable & fixed), maxillofacial prosthetics, implant services, oral and maxillofacial surgery, and orthodontics have no copay.
Home Infusion bundled Services, including Medicare Part B Insulin Drugs, Medicare Part B Chemotherapy/Radiation Drugs, and Other Medicare Part B Drugs, are covered. Medicare Part B Insulin Drugs have a $35 copay, while Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have a coinsurance between 0% and 20%.
Dialysis Services are covered by the Healthy Blue Dual Advantage 2 (HMO D-SNP) plan. There is a 20% coinsurance for dialysis services.
Medical Equipment benefits include Durable Medical Equipment (DME) with no copay and 0-20% coinsurance, Prosthetics/Medical Supplies with no copay and 20% coinsurance, and Diabetic Equipment, including Diabetic Supplies and Diabetic Therapeutic Shoes/Inserts with no copay. Durable Medical Equipment for use outside the home is not covered.
Diagnostic and Radiological Services are covered under the Healthy Blue Dual Advantage 2 (HMO D-SNP) plan. All diagnostic services, diagnostic procedures/tests, and lab services have no copay and a coinsurance of at most 20%, while diagnostic, therapeutic, and outpatient x-ray services have no copay and a coinsurance of at most 20%.
Home Health Services are covered by the Healthy Blue Dual Advantage 2 (HMO D-SNP) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered, but the plan does not cover any of the sub-services including Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, and SET for PAD Services. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered, but additional days beyond Medicare-covered and non-Medicare-covered stays are not covered. Prior authorization is required, and the plan charges the Medicare-defined cost share for tier 1, with no coinsurance information provided.
Other Services include over-the-counter items and meal benefits with no copay, as well as Medicare Community Resource Support with no copay; however, acupuncture and several other services are not covered. Over-the-counter items have a maximum monthly benefit coverage amount of $0.
