Plan Costs:

The cost of a Medicare Advantage Plan is made up of four main parts.

  • First, the monthly premium — the amount you pay every month.
  • Second, the deductible — the amount you pay out of pocket for covered services before the plan starts paying.
  • Third, the copayments and coinsurance — the amounts you pay out of pocket for covered services, usually after meeting the deductible (if applicable). Copays are fixed dollar amounts; coinsurance is a percentage of the cost.
  • Fourth, the Out-of-Pocket Maximum — the maximum amount you could have to pay out of pocket in a year. This may be different for in-network and out-of-network services.

Monthly Premium

The Monthly Premium for this plan is $124.00. This is the amount you must pay every month.

This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.

Deductibles

This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.

This plan has a $590.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.

Out-of-Pocket Maximums

This plan has a combined Maximum Out-Of-Pocket cost of $5700.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $5700.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Regular visits to your primary care doctor are covered and will have a copay of $0.00 and coinsurance of 0% (no coinsurance).

Specialist Visits:

Visits to specialists are covered and will have a copay of $0.00 and coinsurance of 0% (no coinsurance). Specialist visits may require a referral from your primary care doctor or prior authorization.

Emergency Room:

Trips to the Emergency Room are covered, and will have a copay of $130.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

Urgent Care:

Trips to Urgent Care arecovered and will have a copay of $50.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

Drug Coverage IconDrug Coverage

The Blue Cross Medicare Advantage Access (PPO) plan has a $590 deductible for prescription drugs. After you meet your deductible, you will pay a copay or coinsurance depending on the drug tier and the pharmacy you use. For preferred generic drugs, the copay is $10 at a preferred pharmacy and $20 at a standard pharmacy. For standard generic drugs, the copay is $47, regardless of the pharmacy. Preferred brand drugs have a 49% coinsurance, and non-preferred drugs have a 25% coinsurance. Once your total drug costs reach $2000, you enter the catastrophic coverage phase where you pay nothing for covered drugs.

The Blue Cross Medicare Advantage Access (PPO) plan offers comprehensive coverage with a focus on outpatient and preventive services. Many services, including primary care, hearing exams, and eye exams, have no copay. Emergency services, including emergency and urgently needed services, are covered with a copay, and inpatient hospital stays have no copay. This plan includes a wide range of benefits, such as outpatient services, ambulance, and home health services. Some services require copays, such as outpatient services and ambulance services. Diagnostic and radiological services are covered with a combination of copays and coinsurance.

Inpatient Hospital See details

Inpatient Hospital benefits, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, are covered with prior authorization. For both Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, there is no copay for a Medicare-covered stay. Additional Days for Inpatient Hospital-Acute are covered with no copay, while Non-Medicare-covered Stay and Upgrades for Inpatient Hospital-Acute, and Additional Days and Non-Medicare-covered Stay for Inpatient Hospital Psychiatric are not covered.

Outpatient Services See details

Outpatient Services includes coverage for outpatient hospital services with a $325 copay, observation services with no copay, ambulatory surgical center services with a $300 copay, outpatient substance abuse services with a $75 copay for individual and group sessions, and outpatient blood services with no copay. Outpatient services require prior authorization.

Partial Hospitalization See details

Partial Hospitalization is covered by the Blue Cross Medicare Advantage Access (PPO) plan, but requires prior authorization. You will have a $100 copay for this benefit.

Ambulance and Transportation Services See details

Ambulance and Transportation Services are covered by the Blue Cross Medicare Advantage Access (PPO) plan. Ground Ambulance Services have a $275 copay, while Air Ambulance Services have a 20% coinsurance. Transportation Services to any health-related location are not covered.

The Blue Cross Medicare Advantage Access (PPO) plan offers primary care physician services, chiropractic services, occupational therapy, physician specialist services, mental health specialty services, other health care professional services, psychiatric services, physical therapy and speech-language pathology services, additional telehealth benefits, and opioid treatment program services with no copay. Routine chiropractic care and podiatry services are not covered.

Preventive Services See details

Preventive Services are covered, with some services requiring a copay. Medicare-covered preventive services, Kidney Disease Education Services, Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit have no copay. Annual physical exams, health education, in-home safety assessments, personal emergency response systems, medical nutrition therapy, post-discharge in-home medication reconciliation, re-admission prevention, wigs for hair loss related to chemotherapy, weight management programs, alternative therapies, therapeutic massage, adult day health services, nutritional/dietary benefit, home-based palliative care, in-home support services, support for caregivers of enrollees, additional sessions of smoking and tobacco cessation counseling, enhanced disease management, telemonitoring services, home and bathroom safety devices and modifications, and counseling services are not covered. Additional preventive services including Fitness Benefit and Remote Access Technologies (including Web/Phone-based technologies and Nursing Hotline) may have a copay.

Hearing Services See details

Hearing exams, routine hearing exams, and fitting/evaluation for hearing aids are covered with no copay. Prescription hearing aids (all types) are covered with a copay between $699 and $999, while prescription hearing aids for the inner, outer, and over the ear are not covered. OTC hearing aids are not covered.

Vision Services See details

Vision Services includes eye exams and eyewear. Eye exams have no copay, and routine eye exams are covered once per year with no copay. Eyewear has no copay, but contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades are not covered.

Dental Services See details

Dental Services are covered under the Blue Cross Medicare Advantage Access (PPO) plan, with no copay for Medicare Dental Services. Orthodontic Services, Restorative Services, Adjunctive General Services, Endodontics, Periodontics, Prosthodontics (removable and fixed), Maxillofacial Prosthetics, Implant Services, Oral and Maxillofacial Surgery, and Orthodontics are not covered.

Home Infusion bundled Services See details

Home Infusion bundled Services are covered, with prior authorization required. Medicare Part B Insulin Drugs have a $35 copay, with coinsurance between 0% and 20%, while Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have coinsurance between 0% and 20%.

Dialysis Services See details

Dialysis Services are covered under the Blue Cross Medicare Advantage Access (PPO) plan, but require prior authorization. There is a 20% coinsurance for these services.
