Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $100.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
Drugs are not covered by this plan, so a prescription drug deductible is not applicable.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $14000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $14000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Aetna Medicare Eagle Giveback (PPO).
The Aetna Medicare Eagle Giveback (PPO) plan offers comprehensive coverage with varying costs for different services. You will have no copay for primary care, preventive services, and many vision and dental services. However, you can expect copays for inpatient hospital stays, outpatient services, and specialist visits. This plan includes additional benefits such as hearing aids, prescription hearing aids, and coverage for over-the-counter items. Emergency services, ambulance, and home health services are covered as well. Be aware that some services, like cardiac rehabilitation and certain dental and vision services, have limitations or may not be covered.
Inpatient Hospital benefits are covered by the Aetna Medicare Eagle Giveback (PPO) plan, with a copay of $380 for days 1-7 and no copay for days 8-90 for Inpatient Hospital-Acute. Inpatient Hospital Psychiatric has a copay of $407 for days 1-5 and no copay for days 6-90. Non-Medicare-covered Stay and Upgrades for Inpatient Hospital-Acute, and Additional Days and Non-Medicare-covered Stay for Inpatient Hospital Psychiatric are not covered.
Outpatient Services include coverage for all outpatient hospital services, with a copay between $0 and $380 depending on the specific service, and also covers observation services with a $380 copay. Ambulatory Surgical Center (ASC) Services and Outpatient Blood Services have no copay, while Outpatient Substance Abuse Services have a $30 copay for both individual and group sessions.
Partial Hospitalization is covered by the Aetna Medicare Eagle Giveback (PPO) plan. You will have a copay of $80 for this benefit.
Ambulance and Transportation Services are covered by the Aetna Medicare Eagle Giveback (PPO) plan. Ground ambulance services have a $265 copay, while air ambulance services have a 20% coinsurance; however, transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Aetna Medicare Eagle Giveback (PPO) plan. Emergency Services have a $110 copay with no coinsurance, Urgently Needed Services have a $40 copay with no coinsurance, and Worldwide Emergency Coverage, Worldwide Urgent Coverage have a $110 copay with no coinsurance, while Worldwide Emergency Transportation has a $265 copay with no coinsurance.
The Aetna Medicare Eagle Giveback (PPO) plan covers primary care physician services with no copay, chiropractic services with a $15 copay, occupational therapy services with a $20 copay, and physician specialist services with a copay between $0 and $30. Mental health specialty services, psychiatric services, and opioid treatment program services have a $30 copay, and physical therapy and speech-language pathology services have a $20 copay. Additional telehealth benefits have a 20% coinsurance and a copay between $0 and $40.
Preventive services are covered, including annual physical exams with no copay. Additional preventive services include Health Education, Additional Sessions of Smoking and Tobacco Cessation Counseling, Fitness Benefit, Remote Access Technologies, and Wigs for Hair Loss Related to Chemotherapy, all with no copay. Kidney Disease Education Services have a 20% coinsurance. Other preventive services, including Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit, have no copay.
Hearing exams are covered with a $30 copay, and routine hearing exams and fitting/evaluation for hearing aids have no copay. Prescription hearing aids are covered up to a maximum of $500 per year, and prescription hearing aids (all types) have no copay, though prescription hearing aids - inner ear, outer ear, and over the ear are not covered.
Vision services include eye exams and eyewear. Eye exams have a copay of $0-$30. Eyewear, including contact lenses, eyeglasses, and upgrades, has no copay, with a combined maximum plan benefit of $300 per year.
The Aetna Medicare Eagle Giveback (PPO) plan covers dental services with a $2,500 annual maximum, including oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), and oral and maxillofacial surgery with no copay. However, maxillofacial prosthetics, implant services, and orthodontics are not covered.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs with a $35 copay, and Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs with a coinsurance between 0% and 20%. Prior authorization is required.
Dialysis Services are covered by the Aetna Medicare Eagle Giveback (PPO) plan, but require prior authorization. The coinsurance for these services is 20%.
Medical Equipment is covered by the Aetna Medicare Eagle Giveback (PPO) plan, including Durable Medical Equipment (DME), Prosthetics/Medical Supplies, and Diabetic Equipment. DME has a 20% coinsurance with prior authorization required, while Prosthetic Devices and Medical Supplies have a 20% coinsurance, and Diabetic Supplies have a 0-20% coinsurance, and Diabetic Therapeutic Shoes/Inserts have no copay.
The Aetna Medicare Eagle Giveback (PPO) plan covers diagnostic and radiological services, including diagnostic procedures and tests with a copay between $0 and $50, and lab services with no copay. The plan also covers diagnostic radiological services with a maximum copay of $300, therapeutic radiological services with a maximum coinsurance of 20%, and outpatient X-ray services with no copay.
Home Health Services are covered by the Aetna Medicare Eagle Giveback (PPO) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered. Authorization is required for this benefit.
Cardiac Rehabilitation Services are not covered under the Aetna Medicare Eagle Giveback (PPO) plan. While the plan covers Cardiac Rehabilitation Services, the sub-services of Intensive Cardiac Rehabilitation, Pulmonary Rehabilitation, and SET for PAD Services are not covered.
Skilled Nursing Facility (SNF) services are covered by the Aetna Medicare Eagle Giveback (PPO) plan with prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $214.
The Aetna Medicare Eagle Giveback (PPO) plan covers Over-the-Counter (OTC) items with no copay, and a maximum benefit of $30 every three months. The plan also covers a meal benefit with no copay, and other services including annual wellness exams and screening mammography, and gFOBT/FIT with no copay. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
