Monthly Premium
The Monthly Premium for this plan is $30.60. This is the amount you must pay every month.
This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has a $580.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $6200.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $6200.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Wellcare Assist Open (PPO) plan has a $580.00 deductible for prescription drugs. After the deductible, you will pay a copay or coinsurance depending on the drug tier and the pharmacy you use. For example, for preferred generic drugs, you will pay a $19.00 copay at a preferred pharmacy, while standard generic drugs have a 20% coinsurance. Specialty tier drugs have no copay. Once your total drug costs reach $2000.00, you enter the catastrophic coverage phase where you pay nothing for Part D covered drugs.
The Wellcare Assist Open (PPO) plan offers a wide range of benefits with varying costs. Inpatient hospital stays have a copay, while outpatient services have copays ranging from $0 to $300 depending on the service. Primary care visits and many preventive services have no copay, while specialist visits and some therapies have a $20-$25 copay. This plan also covers several other services, including hearing, vision, and dental care, with specific copays or no copays for certain services. Ambulance, emergency, and transportation services have copays, and durable medical equipment and some diagnostic services have coinsurance. Home health and skilled nursing facility (SNF) services are covered, with no copay for home health, and varying copays for SNF stays.
Inpatient Hospital benefits are covered, with a copay of $325 per day for days 1-7 and no copay for days 8-90 for Inpatient Hospital-Acute; Inpatient Hospital Psychiatric has a copay of $275 per day for days 1-7 and no copay for days 8-90. Additional days for Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, Non-Medicare-covered Stay for Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, and Upgrades for Inpatient Hospital-Acute are not covered.
Outpatient services are covered, including outpatient hospital services with a copay between $0 and $300, observation services with a copay between $140 and $300, Ambulatory Surgical Center (ASC) services with a $200 copay, outpatient substance abuse services with a $25 copay for both individual and group sessions, and outpatient blood services with no copay. This plan offers an enhanced benefit of a waived three (3) pint deductible for outpatient blood services.
Partial Hospitalization is covered by the Wellcare Assist Open (PPO) plan, but requires prior authorization. You will have a $130 copay for this benefit.
Ambulance and Transportation Services are covered, including ground and air ambulance services with a $300 copay. Transportation Services to a plan-approved health-related location are covered with no copay, up to 24 one-way trips per year, and includes rideshare services, bus/subway, and medical transport. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Coverage have a copay of $140, $40, and $140 respectively, with no coinsurance. Worldwide Urgent Coverage also has a $140 copay and no coinsurance, while Worldwide Emergency Transportation is not covered.
The Wellcare Assist Open (PPO) plan covers primary care physician services with no copay, chiropractic services with a $20 copay, occupational therapy services with a $20 copay, physician specialist services with a $20 copay, mental health specialty services with a $25 copay for individual and group sessions, and physical therapy and speech-language pathology services with a $20 copay. Additionally, this plan covers other health care professional services with a copay between $0-$20, psychiatric services with a $25 copay for individual and group sessions, additional telehealth benefits with a copay between $0-$40, and opioid treatment program services with a $20 copay. Routine chiropractic care and podiatry services are not covered.
The Wellcare Assist Open (PPO) plan covers preventive services, including an annual physical exam with no copay. Kidney Disease Education Services are covered with a 20% coinsurance. The plan also covers services such as Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit, all with no copay. Other services like Health Education, In-Home Safety Assessment, Medical Nutrition Therapy (MNT), and others are not covered.
Wellcare Assist Open (PPO) covers hearing exams with a $20 copay, routine hearing exams with no copay, and fitting/evaluation for hearing aids with no copay. Prescription hearing aids are covered up to $500 per ear annually with no copay, but inner ear, outer ear, and over the ear prescription hearing aids are not covered, and OTC hearing aids are not covered.
Vision services are covered, including eye exams and eyewear. Eye exams have a copay between $0 and $20, and routine eye exams have no copay. Eyewear, including contact lenses, eyeglasses, eyeglass lenses, eyeglass frames, and upgrades, have no copay and a combined maximum benefit of $300 every year.
Dental Services are covered, including Medicare Dental Services with a $20 copay. Oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), and oral and maxillofacial surgery are covered with no copay, but other services like maxillofacial prosthetics, implant services, and orthodontics are not covered. Orthodontic services have a maximum plan benefit of $3,000 per year.
Home Infusion bundled Services are covered, with prior authorization required. For Medicare Part B Insulin Drugs, there is a $35 copay. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, coinsurance ranges from 0% to 20%.
Dialysis Services are covered by the Wellcare Assist Open (PPO) plan. You will pay 20% coinsurance for these services.
Medical Equipment benefits are covered, including Durable Medical Equipment (DME) with 20% coinsurance, Prosthetics/Medical Supplies with 20% coinsurance, and Diabetic Equipment. Diabetic Supplies have no copay, while Diabetic Therapeutic Shoes/Inserts have 20% coinsurance.
Diagnostic and Radiological Services are covered, with a copay for Diagnostic Procedures/Tests ranging from $0 to $40, and a $0 copay for Lab Services. Diagnostic Radiological Services have a copay of up to $300, Therapeutic Radiological Services have a coinsurance of at least 20%, and Outpatient X-Ray Services have a $25 copay.
Home Health Services are covered by the Wellcare Assist Open (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are not covered by the Wellcare Assist Open (PPO) plan. Although the plan mentions copays, the specific services are not covered, so there will be no cost to you for this benefit.
Skilled Nursing Facility (SNF) services are covered under this plan, but require prior authorization. For days 1-20 and 51-100, there is no copay, while days 21-50 have a copay of $214.
Other Services include coverage for Over-the-Counter (OTC) Items and Meal Benefits. OTC items have no copay, and Meal Benefits also have no copay and require a doctor's referral. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
