This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has a $420.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $6200.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $6200.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Wellcare Simple Open (PPO) plan has a $420.00 deductible for prescription drugs. After the deductible is met, you will pay a copay or coinsurance depending on the drug tier and the pharmacy used. For example, you will pay no copay for preferred generic drugs at preferred or mail-order pharmacies, but a $10.00 copay at standard pharmacies. After your total drug costs reach $2000.00, you enter the catastrophic coverage phase and pay nothing for covered drugs.
The Wellcare Simple Open (PPO) plan offers comprehensive coverage with a variety of benefits. You'll find no copay for many services, including primary care, preventive services like annual physical exams, routine eye exams, and dental cleanings, as well as home health services and OTC items. Other services, like inpatient hospital stays and outpatient services, have copays that vary in price depending on the service, as well as coinsurance in some cases. The plan also offers coverage for hearing and vision services, including hearing exams, hearing aids, and eye exams, with varying copays. Dental services include oral exams, dental x-rays, and cleanings with no copay. Emergency services, ambulance services, and mental health services are also covered. Other notable benefits include home infusion bundled services, dialysis services, and medical equipment.
Inpatient Hospital coverage includes both Inpatient Hospital-Acute and Inpatient Hospital Psychiatric services, both of which require prior authorization. For the first 6 days, there is a $375 copay per admission, and days 7-90 have no copay. Additional days and non-Medicare covered stays are not covered.
Outpatient Services are covered, including outpatient hospital services with a copay between $0 and $300, observation services with a copay between $140 and $300, and ambulatory surgical center services with a $250 copay. Outpatient substance abuse services are covered with a $25 copay for both individual and group sessions, while outpatient blood services have no copay.
Partial Hospitalization is covered by the Wellcare Simple Open (PPO) plan, but requires prior authorization. You will have a $130 copay for this service.
Ambulance and Transportation Services are covered by the Wellcare Simple Open (PPO) plan. Ground and air ambulance services have a copay of $325, and there is no coinsurance. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Coverage are covered under the Wellcare Simple Open (PPO) plan. Emergency Services and Worldwide Emergency Coverage have a $140 copay, while Urgently Needed Services have a $50 copay; all three have no coinsurance. Worldwide Emergency Transportation is not covered.
The Wellcare Simple Open (PPO) plan covers Primary Care Physician Services with no copay, Chiropractic Services with a $20 copay, Occupational Therapy Services with a $35 copay, Physician Specialist Services with a $30 copay, and Mental Health Specialty Services with a $25 copay for individual and group sessions. The plan also covers Other Health Care Professional services with a copay between $0 and $30, Psychiatric Services with a $25 copay for individual and group sessions, and Physical Therapy and Speech-Language Pathology Services with a $35 copay. Additionally, this plan covers Additional Telehealth Benefits with a copay between $0 and $50, and Opioid Treatment Program Services with a $30 copay.
Preventive Services include coverage for Medicare-covered zero-dollar preventive services, annual physical exams with no copay, and additional preventive services. Kidney Disease Education Services are covered with 20% coinsurance, and other preventive services are covered with no copay for glaucoma screening, diabetes self-management training, barium enemas, digital rectal exams, and EKG following Welcome Visit.
Hearing exams are covered with a $30 copay, and routine hearing exams and fitting/evaluation for hearing aids have no copay. Prescription hearing aids are covered up to $500 per ear annually with no copay, while OTC hearing aids are not covered.
The Wellcare Simple Open (PPO) plan covers vision services including eye exams with a copay between $0 and $30, and eyewear including contact lenses, eyeglasses, eyeglass lenses, eyeglass frames, and upgrades with no copay, up to a combined maximum of $200 per year. Routine eye exams are covered with no copay, limited to one visit per year.
The Wellcare Simple Open (PPO) plan covers dental services, including Medicare dental services with a $30 copay. Other services like oral exams, dental x-rays, and cleanings have no copay, while orthodontics are covered up to $1500 per year. However, maxillofacial prosthetics, implant services, and orthodontics are not covered.
Home Infusion bundled Services are covered, but require prior authorization. Medicare Part B Insulin Drugs have a $35 copay. Other Medicare Part B drugs have a coinsurance between 0% and 20%.
Dialysis Services are covered by the Wellcare Simple Open (PPO) plan. The plan has a coinsurance of 20% for dialysis services.
Medical Equipment, including Durable Medical Equipment (DME), Prosthetics/Medical Supplies, and Diabetic Equipment, is covered. DME has a 20% coinsurance, while Prosthetic Devices and Medical Supplies have a 20% coinsurance, and Diabetic Therapeutic Shoes/Inserts have a 20% coinsurance with no copay.
Diagnostic and Radiological Services are covered, including diagnostic procedures/tests with a copay between $0 and $40, and lab services with no copay. Outpatient X-ray services have a $25 copay, while diagnostic radiological services have a copay that is at most $300. Therapeutic radiological services have a coinsurance of at least 20%.
Home Health Services are covered by the Wellcare Simple Open (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered by the Wellcare Simple Open (PPO) plan, but the plan does not cover Cardiac Rehabilitation Services, Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, or SET for PAD Services. Skilled Nursing Facility (SNF) services are covered by the Wellcare Simple Open (PPO) plan, but require prior authorization. For days 1-20 and 51-100, there is no copay, while days 21-50 have a $214 copay; additional and non-Medicare-covered SNF days are not covered.
The Wellcare Simple Open (PPO) plan covers Over-the-Counter (OTC) items with no copay, as well as a meal benefit with no copay and a doctor referral required. Acupuncture, Dual Eligible SNPs, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
