Once your total out-of-pocket costs reach $10100.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The AARP Medicare Advantage from UHC TX-0007 (PPO) plan has a $420 deductible for prescription drugs. After the deductible, your cost will vary depending on the drug tier and the pharmacy you use. For example, you will pay a $12 copay for a standard generic drug at a standard pharmacy. For preferred brand drugs, you will pay a $100 copay. Once your total drug costs reach $2000, you enter the catastrophic coverage phase where you pay nothing for covered drugs.
The AARP Medicare Advantage from UHC TX-0007 (PPO) plan offers a range of benefits with varying cost-sharing. This plan covers inpatient hospital stays with a copay of $325 for the first six days and no copay for the remaining days. Outpatient services have a copay between $0 and $325, and emergency services have a $125 copay. The plan also includes coverage for primary care with no copay, preventive services with no copay, and hearing and vision services with no copay for exams. Dental services are limited to preventive care with no copay. The plan has no copay for home health services, and offers a meal and OTC benefit with no copay.

Outpatient Services include coverage for all outpatient hospital services with a copay between $0 and $325, observation services with a $325 copay, ambulatory surgical center services with no copay, individual outpatient substance abuse sessions with a copay between $0 and $25, group outpatient substance abuse sessions with a $15 copay, and outpatient blood services with no copay. Prior authorization is required for these services.
Partial Hospitalization is covered, but requires prior authorization. You will pay a $55 copay for this benefit.
Ambulance and Transportation Services are covered by the AARP Medicare Advantage from UHC TX-0007 (PPO) plan. Ground and Air Ambulance Services have a copay of $290, with no coinsurance. Transportation Services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the AARP Medicare Advantage from UHC TX-0007 (PPO) plan. Emergency Services have a $125 copay, and Urgently Needed Services have a copay between $0 and $55, while Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation all have no copay.
AARP Medicare Advantage from UHC TX-0007 (PPO) covers primary care physician services with no copay, chiropractic services with a $20 copay, and occupational therapy services with a copay between $0 and $35. This plan also covers physician specialist services with a copay between $0 and $45, and mental health specialty services with a copay between $0 and $25 for individual sessions and $15 for group sessions. Podiatry services, other health care professional, psychiatric services, physical therapy and speech-language pathology services, additional telehealth benefits, and opioid treatment program services are also covered, with varying copays depending on the service.
Preventive Services include coverage for annual physical exams with no copay. Some additional preventive services are covered, but Health Education, In-Home Safety Assessment, Personal Emergency Response System (PERS), Medical Nutrition Therapy (MNT), Post discharge In-Home Medication Reconciliation, Re-admission Prevention, Wigs for Hair Loss Related to Chemotherapy, Weight Management Programs, Alternative Therapies, Therapeutic Massage, Adult Day Health Services, Nutritional/Dietary Benefit, Home-Based Palliative Care, In-Home Support Services, Support for Caregivers of Enrollees, Additional Sessions of Smoking and Tobacco Cessation Counseling, Enhanced Disease Management, Telemonitoring Services, Remote Access Technologies (including Web/Phone-based technologies and Nursing Hotline), and Counseling Services are not covered.
Hearing exams are covered with no copay, but routine hearing exams are limited to one per year. Prescription hearing aids are partially covered, with a copay between $199 and $1249 for all types of prescription hearing aids; however, inner ear, outer ear, and over the ear hearing aids are not covered. OTC hearing aids are covered with a copay between $99 and $829.
The AARP Medicare Advantage from UHC TX-0007 (PPO) plan covers vision services, including eye exams with no copay, and routine eye exams with no copay for one visit every year. Eyewear is covered, including contact lenses, eyeglass lenses, and eyeglass frames, with a combined maximum benefit of $200 every two years; however, eyeglasses (lenses and frames) and upgrades are not covered.
Dental services include coverage for oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, and other preventive dental services. Oral exams, dental x-rays, prophylaxis (cleaning), fluoride treatment, and other preventive dental services have no copay. All other dental services are not covered.
Home Infusion bundled Services are covered, with prior authorization required. Medicare Part B Insulin Drugs have a $35 copay, and a coinsurance between 0% and 20%. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have a coinsurance between 0% and 20%.
Dialysis Services are covered, but require prior authorization. You will pay 20% coinsurance for this benefit.
Medical Equipment, including Durable Medical Equipment (DME), is covered with 20% coinsurance, while Prosthetics and Medical Supplies are covered with a 20% coinsurance. Diabetic Supplies have no copay, and Diabetic Therapeutic Shoes/Inserts have 20% coinsurance.
Diagnostic and Radiological Services include coverage for all diagnostic services with a $30 copay for diagnostic procedures and tests, and no copay for lab services. Diagnostic Radiological Services have a copay of up to $210, while Therapeutic Radiological Services have 20% coinsurance, and Outpatient X-Ray Services have a $20 copay.
Home Health Services are covered by the AARP Medicare Advantage from UHC TX-0007 (PPO) plan with no copay and no coinsurance. However, additional hours of care and personal care services are not covered.
Cardiac Rehabilitation Services are covered, but the specific services of Cardiac Rehabilitation, Intensive Cardiac Rehabilitation, Pulmonary Rehabilitation, and SET for PAD are not covered. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered, with prior authorization required. You will have no copay for days 1-20, and a $203 copay for days 21-100.
Other Services include coverage for Over-the-Counter (OTC) Items and a Meal Benefit. The OTC benefit has no copay, while the Meal Benefit also has no copay and requires prior authorization. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, and many other services are not covered.
