Monthly Premium
The Monthly Premium for this plan is $50.60. This is the amount you must pay every month.
This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has a $590.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $9350.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan has a $590 deductible for prescription drugs. After the deductible is met, you will pay costs for drugs in each tier until your total drug costs reach $2000, at which point you enter the next coverage phase. This plan's premium may be reduced if you qualify for the low-income subsidy (LIS). If you qualify for LIS, the monthly premium for Part D is $50.60. Once your yearly out-of-pocket drug costs reach $2000, you will pay nothing for covered drugs.
The Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan offers a range of benefits with varying cost-sharing. Many services have a 20% coinsurance, including outpatient, primary care, and vision services. Emergency services come with a copay, while other services like ambulance, transportation, and home health services have no copay. The plan also includes coverage for hearing aids and dental services. Hearing aid benefits cover up to $3,000 per year for prescription hearing aids and $300 per year for OTC hearing aids, with no copay. Dental services have a $6,000 maximum benefit per year, with no copay for many services.
Inpatient Hospital benefits, including Acute and Psychiatric, are covered, but additional days, non-Medicare-covered stays, and upgrades are not covered. For covered services, the plan uses the Medicare-defined cost share for tier 1, with coinsurance details available in the plan documents.
Outpatient services, including outpatient hospital services, observation services, and outpatient substance abuse services, are covered with a 20% coinsurance. Outpatient blood services have no copay.
Partial Hospitalization is covered by the Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan with a $55 copay, and prior authorization is required.
The Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan covers ambulance services with a 20% coinsurance for both ground and air ambulance services, and transportation services with no copay for plan-approved health-related locations up to 150 one-way trips per year using rideshare services, bus/subway, van, or medical transport. Transportation services to any health-related location is not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services have a $90 copay, Urgently Needed Services have a $20 copay, and Worldwide Emergency Services have a $0 copay for Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation.
Primary Care Physician Services, Chiropractic Services, Occupational Therapy Services, Physician Specialist Services, Mental Health Specialty Services, Podiatry Services, Other Health Care Professional, Psychiatric Services, Physical Therapy and Speech-Language Pathology Services, Additional Telehealth Benefits, and Opioid Treatment Program Services are covered under the Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan. Primary Care Physician Services, Chiropractic Services, Occupational Therapy Services, Physician Specialist Services, Mental Health Specialty Services, Podiatry Services, Other Health Care Professional, Psychiatric Services, and Physical Therapy and Speech-Language Pathology Services have a 20% coinsurance. Additional Telehealth Benefits have no copay.
The Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan covers preventive services, including an annual physical exam with no copay. Additional preventive services, such as Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit, are covered with 20% coinsurance.
Hearing services include hearing exams, routine hearing exams, fitting/evaluation for hearing aids, prescription hearing aids, and OTC hearing aids. Routine hearing exams and fitting/evaluation for hearing aids have no copay, and routine hearing exams have a 20% coinsurance. Prescription hearing aids have no copay and are covered up to $3,000 per year for both ears combined, while OTC hearing aids have no copay and are covered up to $300 per year for both ears combined. Prescription Hearing Aids - Inner Ear, Prescription Hearing Aids - Outer Ear, and Prescription Hearing Aids - Over the Ear are not covered.
The Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan covers vision services, including eye exams with 20% coinsurance, and eyewear with 20% coinsurance and a combined maximum benefit of $650 per year. Routine eye exams have no copay, and contact lenses, eyeglasses (lenses and frames), and eyeglass lenses have no copay, but upgrades are not covered.
Dental Services include coverage for Medicare Dental Services with 20% coinsurance, and other dental services with a $6,000 maximum benefit per year. Oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, maxillofacial prosthetics, implant services and oral and maxillofacial surgery have no copay. Orthodontics is not covered.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs with a $35 copay, as well as Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs with 0-20% coinsurance. Prior authorization is required.
Dialysis Services are covered under the Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan. The coinsurance for these services is 20%.
Medical Equipment benefits include Durable Medical Equipment (DME) with no copay and 0-20% coinsurance, Prosthetic Devices with 20% coinsurance, and Medical Supplies with 20% coinsurance. Diabetic Equipment includes no copay for Diabetic Supplies and Diabetic Therapeutic Shoes/Inserts. Durable Medical Equipment for use outside the home is not covered.
Diagnostic and Radiological Services are covered under the Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan. Diagnostic procedures/tests and lab services have a coinsurance of at most 20%, while diagnostic and therapeutic radiological services, and outpatient X-ray services also have a coinsurance of at most 20%.
Home Health Services are covered by the Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are not covered by the Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan. While some services are listed as covered, none of the sub-services are actually covered.
Skilled Nursing Facility (SNF) services are covered, but the plan does not cover additional days beyond Medicare-covered for SNF or non-Medicare-covered stays. Prior authorization is required, and you will pay the Medicare-defined cost share for tier 1 with this plan.
The Wellpoint Full Dual Advantage 2 (HMO-POS D-SNP) plan covers over-the-counter items and meal benefits with no copay. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
