Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The AARP Medicare Advantage Essentials from UHC CO-4 (HMO-POS) plan has a $340 deductible for prescription drugs. After the deductible is met, you will pay a copay or coinsurance depending on the drug tier and the pharmacy used. For example, you will pay an $8 copay for preferred generic drugs at a standard pharmacy, and $47 for standard generic drugs. For preferred brand drugs, the copay is $100. Once your total drug costs reach $2000, you enter the catastrophic coverage phase where you pay nothing for covered drugs.

Outpatient Services include coverage for all outpatient hospital services with a copay between $0 and $320, observation services with a $320 copay, ambulatory surgical center services with no copay, outpatient substance abuse services with a copay between $0 and $25 for individual sessions and a $15 copay for group sessions, and outpatient blood services with no copay. Prior authorization is required for some services.
Partial Hospitalization is covered by the AARP Medicare Advantage Essentials from UHC CO-4 (HMO-POS) plan, and requires prior authorization. You will pay a $55 copay for this benefit.
Ambulance and Transportation Services are covered by the AARP Medicare Advantage Essentials from UHC CO-4 (HMO-POS) plan. Ground and Air Ambulance Services have a $150 copay, and there is no coinsurance. Transportation Services to any health-related location are not covered.
Emergency Services are covered with a $125 copay, and no coinsurance. Urgently Needed Services have a copay between $0 and $55, and no coinsurance. Worldwide Emergency Services, Worldwide Urgent Coverage, and Worldwide Emergency Transportation all have no copay and no coinsurance.
Primary Care Physician Services are covered with no copay. Chiropractic Services, including routine care, have a $10 copay and require prior authorization. Occupational Therapy Services, Physician Specialist Services, Mental Health Specialty Services, Podiatry Services, Other Health Care Professional, Psychiatric Services, Physical Therapy and Speech-Language Pathology Services, Additional Telehealth Benefits, and Opioid Treatment Program Services are also covered, with varying copays.
Preventive services are covered, including an annual physical exam with no copay. Additional preventive services like Health Education, In-Home Safety Assessment, and others are not covered.
Hearing exams are covered with no copay, and routine hearing exams are covered once per year with no copay. Prescription hearing aids are covered with a copay between $199 and $1249, and OTC hearing aids are covered with a copay between $99 and $829. Fitting/Evaluation for Hearing Aid, Prescription Hearing Aids - Inner Ear, Prescription Hearing Aids - Outer Ear, and Prescription Hearing Aids - Over the Ear are not covered.
The AARP Medicare Advantage Essentials plan covers vision services, including eye exams with no copay, and eyewear with no copay, up to a combined maximum of $200 every two years; however, eyeglasses (lenses and frames) and upgrades are not covered. Routine eye exams and contact lenses are covered with no copay. Eyeglass lenses have a copay between $0 and $153 per pair.
Dental services are covered, including oral exams, dental x-rays, prophylaxis (cleaning), fluoride treatment, and other preventive dental services, with no copay for most services and a 20% coinsurance for Medicare dental services. Orthodontic services, restorative services, and more are not covered.
Home Infusion bundled Services are covered, with prior authorization required. Medicare Part B Insulin Drugs have a $35 copay and 0-20% coinsurance, while Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have 0-20% coinsurance.
Dialysis Services are covered by the AARP Medicare Advantage Essentials from UHC CO-4 (HMO-POS) plan and require prior authorization. The coinsurance for Dialysis Services is 20%.
Medical Equipment is covered, including Durable Medical Equipment (DME), Prosthetics/Medical Supplies, and Diabetic Equipment. DME has a 20% coinsurance and requires authorization, while Prosthetic Devices, Medicare-covered Prosthetic Devices, and Medicare-covered Medical Supplies have a 20% coinsurance. Diabetic Supplies have no copay, and Diabetic Therapeutic Shoes/Inserts have a 20% coinsurance.
Diagnostic and Radiological Services include coverage for Diagnostic Procedures/Tests with a $50 copay, Lab Services with no copay, Diagnostic Radiological Services with a copay up to $135, Therapeutic Radiological Services with 20% coinsurance, and Outpatient X-Ray Services with a $25 copay. Prior authorization is required for all diagnostic and radiological services.
Home Health Services are covered by the AARP Medicare Advantage Essentials from UHC CO-4 (HMO-POS) plan with no copay and no coinsurance. Additional hours of care and personal care services are not covered.
Cardiac Rehabilitation Services are covered, but the plan does not cover Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, or SET for PAD Services. Prior authorization is required for this benefit.
Skilled Nursing Facility (SNF) services are covered by the AARP Medicare Advantage Essentials from UHC CO-4 (HMO-POS) plan, but require prior authorization. You will have no copay for days 1-20, and a $203 copay for days 21-100. Additional days beyond Medicare-covered for SNF and Non-Medicare-covered stays for SNF are not covered.
Under "Other Services", acupuncture, meal benefits, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, private duty nursing services, case management (long term care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered. Over-the-Counter (OTC) Items are covered with no copay.
