Monthly Premium
The Monthly Premium for this plan is $21.30. This is the amount you must pay every month.
This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has a $590.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $9350.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The Wellcare Dual Access (HMO-POS D-SNP) plan has a $590 deductible for prescription drugs. After the deductible is met, you will pay the costs for drugs in each tier until your total drug costs reach $2000, at which point you enter the next coverage phase. This plan's premium may be reduced if you qualify for the low-income subsidy (LIS). The monthly premium for Part D is $21.30.
The Wellcare Dual Access (HMO-POS D-SNP) plan offers a range of benefits with varying cost-sharing. Hospital stays have a $1,810 copay per admission, while outpatient services, including blood services, have a 20% coinsurance. Ambulance services also have a 20% coinsurance, and emergency services have copays ranging from $45 to $110. Primary care, preventive services, hearing, vision, and dental services are available. Many services, such as routine hearing exams and eyewear, have no copay. The plan also covers home health services with no copay and no coinsurance.
Inpatient Hospital coverage includes both Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, each with a copay of $1,810 per admission or stay for Medicare-covered services. Additional days, and non-Medicare-covered stays, and upgrades for Inpatient Hospital-Acute and Inpatient Hospital Psychiatric are not covered.
Outpatient Services include coverage for outpatient hospital services with a 20% coinsurance and no copay, observation services with a 20% coinsurance, ambulatory surgical center services with a 20% coinsurance, outpatient substance abuse services with a 20% coinsurance, and outpatient blood services with a 20% coinsurance. The plan waives the three-pint deductible for outpatient blood services.
Partial Hospitalization is covered by the Wellcare Dual Access (HMO-POS D-SNP) plan, but requires prior authorization. You will pay a 20% coinsurance for this benefit.
Ambulance and Transportation Services are covered by the Wellcare Dual Access (HMO-POS D-SNP) plan. Ground and air ambulance services have a 20% coinsurance, and transportation services to plan-approved health-related locations have no copay, with a limit of 24 one-way trips per year.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Coverage all have a copay of $110, $45, and $110, respectively, with no coinsurance. Worldwide Urgent Coverage has a copay of $110, with no coinsurance, and Worldwide Emergency Transportation is not covered.
Primary Care benefits under the Wellcare Dual Access (HMO-POS D-SNP) plan include coverage for Primary Care Physician Services with 20% coinsurance, Chiropractic Services with 20% coinsurance (but not routine care), Occupational Therapy Services with 20% coinsurance, Physician Specialist Services with 20% coinsurance, Mental Health Specialty Services with 20% coinsurance, Physical Therapy and Speech-Language Pathology Services with 20% coinsurance, Other Health Care Professional with 20% coinsurance, Psychiatric Services with 20% coinsurance, Additional Telehealth Benefits with a $0-$45 copay and 20% coinsurance, and Opioid Treatment Program Services with 20% coinsurance; Podiatry Services are not covered.
Preventive services, including annual physical exams, are covered with no copay. Additional preventive services, including Medicare-covered Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit, have a 20% coinsurance.
Hearing exams are covered, with a coinsurance of at most 20% for routine hearing exams, and fitting/evaluation for hearing aids has no copay. Prescription hearing aids are covered with a maximum benefit of $1000 per year, with no copay for all types of prescription hearing aids, but inner ear, outer ear, and over the ear hearing aids are not covered.
Vision Services includes coverage for eye exams, with a 20% coinsurance and no copay, and eyewear with a 20% coinsurance. Routine eye exams have no copay and are limited to one per year, and contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades have no copay.
The Wellcare Dual Access (HMO-POS D-SNP) plan covers dental services, including Medicare dental services with 20% coinsurance and other dental services with no maximum plan benefit coverage. Other covered services include oral exams, dental x-rays, and other diagnostic dental services with no copay, as well as prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), and oral and maxillofacial surgery, all with no copay. Orthodontic services have a maximum plan benefit of $4000 per year. However, maxillofacial prosthetics, implant services, and orthodontics are not covered.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs with a $35 copay, and Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs with 0-20% coinsurance. Prior authorization is required for these services.
Dialysis Services are covered under the Wellcare Dual Access (HMO-POS D-SNP) plan. The plan has a coinsurance of 20% for these services.
Medical Equipment benefits include Durable Medical Equipment (DME) with 20% coinsurance, Prosthetics/Medical Supplies with 20% coinsurance, and Diabetic Equipment with 20% coinsurance for covered services; however, Durable Medical Equipment for use outside the home is not covered. There is no copay for any of these services.
Diagnostic and Radiological Services are covered under the Wellcare Dual Access (HMO-POS D-SNP) plan. Diagnostic Procedures/Tests and Diagnostic Radiological Services have a coinsurance of at most 20%, and Lab Services have no copay and a coinsurance of at most 20%. Therapeutic Radiological Services and Outpatient X-Ray Services have a coinsurance of at most 20%.
Home Health Services are covered under the Wellcare Dual Access (HMO-POS D-SNP) plan with no copay and no coinsurance. However, Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are not covered by the Wellcare Dual Access (HMO-POS D-SNP) plan. Although the plan covers some cardiac rehabilitation services, the specific services listed are not covered, so the benefit is effectively not covered.
Skilled Nursing Facility (SNF) services are covered, but require prior authorization. There is no copay for days 1-20, but there is a $214 copay per day for days 21-100.
The Wellcare Dual Access (HMO-POS D-SNP) plan does not cover acupuncture, meal benefits, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, or Self-Directed Personal Assistance Services. Over-the-Counter (OTC) Items are covered with no copay.
